CENSUS FORMS
This year we are required by the federal government to collect information on Ethnicity/Race.The changes will allow our students to more accurately describe who they are. This information will not be reported to any federal agency in a way that identifies your child.This is very important, if you do not fill out this section on the census form and return to us, we will be contacting you until this information is completed for each student.

Census forms contain an open campus permission statement for students in grades 11-12. Parents need to check this statement and have students return their completed census forms on “Back to School” Readiness Days.This form MUST be turned in before your student can go through Readiness Days. Any 11th and 12th grade student NOT returning their Census form will automatically be tagged LOC and will not be allowed to leave campus for lunch.

A reminder when filling out the census forms: Be sure to include or update your e‑mail address. We would like to emphasize the importance of the e-mail address.This is a key resource for getting in contact with you during the school year.
